Please bring the following items to your first appointment:
Photo ID
Dental insurance card, if applicable
Any recent dental X-rays, if available
Referral form, if your dentist provided one
List of current medications or medical conditions
Parent or guardian, if the patient is under 18

During your first visit, we will review your dental and medical history, examine your teeth and bite,
and discuss your orthodontic concerns.
If needed, we may take photos, digital scans, or X-rays to better evaluate your teeth, jaw alignment,
and treatment options.
After the examination, Dr. Choe will explain whether orthodontic treatment is recommended and discuss possible options such as braces, clear aligners, or other treatment plans.

QUÉ DEBE TRAER
Por favor traiga lo siguiente a su primera cita:

QUÉ PUEDE ESPERAR
Durante su primera visita, revisaremos su historial dental y médico, examinaremos sus dientes y mordida, y hablaremos sobre sus preocupaciones de ortodoncia.
Si es necesario, podemos tomar fotos, escaneos digitales o radiografías para evaluar mejor sus dientes, la alineación de la mandíbula y sus opciones de tratamiento.
Después del examen, el Dr. Choe le explicará si se recomienda tratamiento de ortodoncia y hablará sobre posibles opciones como brackets, alineadores transparentes u otros planes de tratamiento.
